Grading of Laryngeal Stenosis

Grade 1

Less than 70% obstruction


Grade 2

Between 70% and 90% obstruction


Grade 3

Tight subglottic stenosis with pinhole opening Greater that 90% obstruction Recognizable vocal cords

Tight glottic and subglottic stenosis with 90% obstruction. Vocal cords not recognizable

Posterior glottic scarring, with tethering of the posterior glottic structures. Note the absence of a posterior mucosally lined tract, which differentiates this lesion from webbing between the vocal processes of the arytenoids.


Grade 4

Complete laryngeal obstruction with no recognizable true vocal cords and no lumen.
