I was just wondering what criteria are used to determine when you can start using a PMV? Hannah is "laughing" now and I'm dying to hear her. About the only sound that comes from her is when she is crying...and that just sounds pathetic...just her forcing air out of the trach.

Anyway, I was just curious how soon your kids were allowed to use the PMV and under what circumstances.

Patricia was trached at 5 yrs old, and the same week they put in the trach they started to try the PMV. In the long run Patricia has not liked it (because of VC issues) and has mastered just covering her HMV with her hand to increase her vocal power.

She is still on oxygen. Does that matter? (I feel so ignorant sometimes! )

When Jacob was about 6 months old, I asked his (first) pulmonologist to write an order for a PMV. He said no, because Jacob was having a difficult time with just an HME. However, looking back, Jacob just had a difficult entire 1st year without the vent support. Once he began getting the nocturnal vent support around 11 months old, we started trying to use the PMV with the blessing of a much better pulmonologist. It tooks a few months of a few minutes here, few minutes there, but finally he began wearing it all day at around 16 months. And he still does, except when he is sick.
