Reconstructive Surgeries
Hello all! Just doing some some research on airway reconstruction. Wondering if you all have any experience with (1) crychoid split; (2) resection; and (3) LTP? I believe we will be a candidate for #3 due to Hudson's stenosis being extremely close to his vocal chords. What is the procedure before, during and after? Recovery time? Also, will he have the trach after LTP for a short period of time?

Spent 9 weeks in the NICU before coming home on his due date. Trached 10/11/09, g-tube on 3/4/10, double stage LTP on 3/11/11. Awaing next steps!

Sydney had the LTP done. You must get it done at Cincinnati with Dr. Rutter or Cotton - they do it a bunch of different ways. Sydney had a staged surgery - we had to go out there a whole bunch of times.

mother of Sydney, born 8/17/06 (27 weeks gestational age). Trached for almost three years. She had two fundoplications, PDA ligation, and a rib graft surgery done at Cincinnati. She was decannulated on July 2nd.

i thought that for the resection the stenosis had to be a certain distance AWAY from the cords...maybe i remembered that wrong.

My son had a single stage LTR that became a double stage LTR this summer. I think an LTR and and LTP are one in the same thing (but correct me someone if I'm wrong here). Single stage recovery is longer, and involves at least a week in PICU, but, if it works, you get the trach out a lot quicker too.
I think resection involves removing part of the trachea, which is riskier if if does not work.

Laura, Mum to Dominic, born full term in September 2005. Trach at six weeks due to a subglottic haemangioma and acquired subglottic stenosis. LTR in August 2009 and decannulated February 2010. A few other issues, that seem to be slowly resolving.

In the way you are using it, I do think the terms mean the same thing.
If the "R" LTR stands for "reconstruction" then LTR is a more generic term that encompasses LTP. Some medicos use LTR instead of LTP in that case.
I also think, though, that some folks use it to mean "resection". My advice (worth what y'all paid for it) is that we should probably try to define our terms when we use them just to be sure we are all talking about the same thing.

Hi there--
I just wanted to say that our little guys share a birthday, and a grade three stenosis. My guy was born in 08, though.
We have been to Cincy and are awaiting a date to have a single-stage LTP. Officially, a laryngotracheoplasty with an anterior graft using rib cartilage.
We were told that in our case, a cricoid split wouldn't be possible because of his size. Not sure what that means, because I'm pretty sure I've heard of it done on tiny babies. Maybe they meant the size of his airway?
Anyway, not trying to muddy the waters. I just wanted to say hello. It seems we have a little bit in common!
