Default Ulcer near stoma...Pictures

Tommy has developed some sort of ulcer above his stoma. It sits under the center of the trach tube, so that when I am cleaning, I practically have to decan him to clean it and apply triple antibiotic cream. I've been doing this for 3 days now a couple of times a day, and I am keeping split gauze between it and his tube. I hope this helps it. I don't know what else I can do. I do not want an infection that close to his trach! When he woke up this morning, the gauze had normal secretions soaking it, but I noticed a few spots of green and a couple drops of blood on the gauze as well. He's been on amoxicillian all last week. I'm tempted to give him the rest of the bottle just to keep this thing from growing something nasty.

Anybody ever see this before??? What caused it and what else can I do?

Are the trach ties too tight? I would get him to the doctor to check this.

Yes, I would definitely get a doctor to look at that. Can you try a new trach tube? sometimes I have noticed that the plastic can be a bit sharp and it could be digging into his neck.

I can put the tip of my finger under the ties all the way around, so I know they are not too tight. I will try changing the tube tomorrow when we do trach care, and I will call his doctor. Thanks!

You might ask your child's doctor for a consult with an ostomy/wound nurse? Because it's so close to the trach stoma, maybe an ostomy nurse might have some good suggestions.

Pressure sore!! We're having the same problem right now.

Elijah won't let us keep any padding under the flange or he'll rip it out along with the trach.

Duoderm works well. I'm trying to reorder that but everyone is out around here.

Both twins have it. Ooooh! I'll take a pic too!

Ahhh! Makes sense! I emailed the pictures to our ENT's nurse. He happens to be in a satellite clinic today, so he is going to see them first thing in the morning. Hopefully he can call us in a prescription for the duoderm. I've been putting split gauze under his trach for the past couple of days, but it doesn't seem to be helping any. Gosh, I almost feel like I need to get a trach tube that doesn't sit on his skin for a few days. Don't know how that one would work!

Oh yeah, we've had the same thing here when Celia was a baby! Those chubby little necks are cute but can really mess things up. Keep it really dry,use guaze or Duoderm, I used bactroban on Celia's and that seemed to work!! good luck!
