That is exactly what Jake does. He likes vocalizing, but as soon as he coughs, it starts an escalation that ends up in him vomiting. Our ENT said that some kids just don't tolerate the PMV that well, even with a hole drilled in it. Since Jake is plugging his trach with his finger when he wants to vocalize, the ENT said it's no big deal--he'll be just one of the kids that uses his finger instead of the PMV, but we do wish there was something we could do.
