While Tommy's on the vent, we use Portex Thermovent 600's by Smiths. He's fine with that while on the vent.

When he's off the vent, we have tried Portex Thermovent T also by Smiths. He starts coughing and working hard to breath and needs suctioning every 5 minutes. Take him off his HME and he does dry out to the point where I know he really needs an HME, but there is no increased work of breathing. So, we leave the HME off and let him have his freedom knowing we just have to lavagie him more often. But we've also noticed as the day goes on, his suction canister gets this tanish/orangish color to it. He always has stuff growing in his trache. Docs think it's just colonized, and I would have to agree because he has no other symptoms beside a yuky canister, but when he does get an active infection he gets many other symptoms and it's clear he's ill. This is not the case this time!

So, we tried these new HME's that have green sponges, 1870 Hydro-Trach T, but these things are MASIVELY HUGE!!!! Tommy can't even look down to see what his hands are doing or where his feet are going! He hates them and rips them off to the point where he nearly rips his trache out!

I know some of you said your children tolerate the "sponge" HMEs better than the paper filtered HMEs, but which "sponge" HMEs do you use?

This is the one I've found Ainsley to tolerate the best.http://www.tracoe.com/produkte/9/102/tracoe-humid-assist-i.html And I like that I can sterilize and reuse.

We've used trachphone for over two year's now and have had no problems. We just change the HME daily, or more if having an extra junky day.

We just started using these about 6 weeks ago and LOVE, LOVE, LOVE them. We used different hme's with paper while off the vent and this is by far the best. It is a throw away kind but uses a sponge like like filter. It really gives her the best amout of moisture, not junky and not dry. I can't say enough how good this had been for her. O2 can also be hooked up if needed.

Side note, our DME just started carrying these.

Leave it to this forum to give me the information I was looking for! I never knew there were so many kinds of HME's! Thanks so much! We're contacting the manufactures now for some samples before going to our DME. There's bound to be one that works for Tommy :D

I use the Humidvent mini. The "T" HME's are too bulky for Addisyn's chubby neck and pops off when she moves around.

http://www.smiths-medical.com/plugins/news/2006/oct/thermovent-t2-hme.html These are intriguing. It is amazing how you find out about new products here. I would love to try these T2's since they have the suction port. Thanks for the tip.

We have used hme 1870 'green' sponge swedish noses for 4 years.... in conjunction with the PMV when able.

My understanding was the green sponges provide some natural humidification. Also, if your child needs O2 support you would have to use these? They have the plug to attach whereas the white swedish noses have no extra humidification and no oxygen adaptor.

You may find these big, but they are better than a vent! I love the way that you can suction via the flap, unlike the white ones. So, we actually tie them on.. We use the blue trache ties with velcro and add a 2nd one over the top of the hme so that it doesn't fall off easily.

Secretion wise, we wait about 30 mins after waking before changing from the vent to HME (although now Mitch is bigger we don't wait this long). Once secretions are lower then we switch to the PMV.
Cheers Cherie
